Appointment Information for New Patients
Please arrive at the office 20 minutes early
In order to make sure your NEW PATIENT visit with Shore Pulmonary is successful we ask that you gather the following information ( if applicable) and please bring copies that we may keep , with you for your appointment.
- Valid form of I.D.
- Insurance card(s)
- Referral
- Completed paperwork
- Co-pay due at time of service
- Chest x-ray (s), chest CT scans, PET scan, cardiology work-up, recent blood work, EKG
Please Note: If you are unable to keep your scheduled appointment, kindly give us 24 hours notice. This will allow us to accommodate other patients needing an appointment. A cancellation fee of $25.00 will be charged for appointments not cancelled 24 hours in advance.
